To request treatment authorization, send your request to the dedicated UR fax number of the regional office handling the claim.

SINGLE REMITTANCE
To receive payment at an address different from your tax reporting address, you must complete SCIF Form 5159. See the Single Remittance Address Policy for more information and link to form.
