Prepaid Visa® Debit Card

U.S. Bank Prepaid VISA Debit Card*

Eligible claimants will receive a letter in the mail with a unique code to use to enroll in our debit card program for a fast, secure, and convenient way to receive workers’ compensation disability payments!

Step 1:

Review the U.S. Bank Pre-Acquisition Disclosures.

Step 2:

Enroll using your unique code in the online portal.
If you have previously enrolled, you may opt-out of the program.

The debit card makes accessing temporary disability, permanent disability, and life pension payments easy and your funds are available much sooner than with a paper check.

You can use it wherever Visa is accepted, to withdraw cash from in-network ATMs, or make point-of-sale purchases without incurring fees.

Using the card can save you trips to the bank and remove the risk of losing or damaging a check.

In addition, you can withdraw the entire balance from the debit card without incurring fees.

If you lose or misplace your card, or believe there has been unauthorized use, you must call (866) 936-0315 immediately to report the loss of the card or any unauthorized use. You are generally protected from liability for unauthorized transactions with Zero Liability. Certain limitations and conditions may apply. See your Cardholder Agreement for details. The Cardholder Agreement supersedes this notice.

State Fund is committed to fighting fraud. We thoroughly vet all workers’ compensation claims we receive to ensure their validity before distributing benefits, both medical and indemnity.

Should you experience any difficulty accessing your funds, please call your State Fund claims adjuster.

Please Note: State Fund has no access to your debit card account balance or purchase activity.
