<h1>File a Claim</h1>
<p>Filing a claim quickly after a workplace injury occurs can help ensure any needed treatment can be provided timely, which will improve outcomes for the injured worker.</p>
<p>If you are the injured worker, we have <a href="/employees/file-claim-if-injured.asp">developed resources to assist you</a> in the claims process. </p>

<p>In addition to ensuring the best outcome, some steps are required to be completed within certain timeframes after a work injury occurs or is known.</p>
<ul>
<li><a href="/pdf/3067.pdf">Employer’s Report of Occupational Injury or Illness 3067</a> must be submitted no later than five days after the injury or illness occurs or is known.</li>
<li>Here are <a href="/forms/forms.asp">all required forms</a>.</li>
<li><a href="https://www.dir.ca.gov/dosh/report-accident-or-injury.html">Cal/OSHA</a> must be notified within 24 hours if the injury is serious or results in death.</li>
</ul>
<h2>Getting treatment for your injured worker</h2>
<p>Immediate treatment of the injury could include calling 911, providing first aid, and/or sending the injured worker to a physician in <a href="/claims/MPN.asp">State Fund’s Medical Provider Network</a> or their predesignated physician.</p>

<h2>Three ways to file a claim</h2>
<h3><strong>Online</strong></h3>
<p>File through <a href="https://statefundonline.scif.com/">State Fund Online</a>. </p>
<p> Claims can be filed any time 24/7. Besides filing a claim, State Fund Online provides tools to manage your claim and communicate directly with your claims adjuster. </p>
<p> If you bound your policy with us through <a href="https://www.StateFundDirect.com">www.StateFundDirect.com</a>, you will use the username and password you created there to access State Fund Online.</p>

We will refer you to a physician and will provide you written copies of all documents by mail or fax.</p>
