State Fund does not sell, trade, or otherwise transfer your private or  confidential  information or general information to third parties, except as permitted or required by law to conduct its workers’ compensation insurance business.  State Fund provides some of its services through contractual arrangements with medical and legal service providers, brokers and other third parties (collectively, “service partners”).  State Fund’s service partners may use your private or confidential information to operate State Fund’s website and to deliver their services. These parties will only use your data for the purpose of carrying out the work as agreed with State Fund, and are required to keep the information confidential.  However, the use of your private or confidential information by State Fund’s service partners is governed by law and the privacy policies of those service partners, and is not subject to State Fund’s control.

State Fund will release private or confidential information without your consent:

1. For security purposes, to prevent imminent harm to any person or entity.

3. For transmittal to other State agencies, including the Workers’ Compensation Board, Department of Insurance, and other entities as provided for by law.

Request to correct private or confidential information shall be responded to within a reasonable time.

If you request it, State Fund will provide you with a notice of your right to prevent sharing of your private or confidential information to the extent permitted or required by law, as well as a cost-free means of doing so. • For Employees at State Fund – Information that identifies or describes an individual including but not limited to his or her social security number, physical description, home address, home telephone number, education, financial matters, and medical and employment history.  It includes statements made by, or attributed to, the individual. 
• For Policyholders, Claimants, and Third parties – Any individually identifiable information gathered in connection with an insurance transaction from which judgments can be made about an individual’s character, habits, avocations, finances, occupation, or an individual’s name and address and medical information.

2. Confidential Information – information about State Fund, a State Fund policyholder, or claimant, or a third party of such nature that it has independent economic value from not being known to the public.

• State Fund policyholder “confidential information” includes, but is not limited to, wage, salary and tax information; trade secrets; financial statements, credit reports, and any other information that the insured has lawfully designated proprietary or trade secret.
